Your Patient Care Journey

Referral
Your PCP or a case manager from a skilled nursing facility, hospital or home health agency (or other healthcare provider) will contact Woundtech with a referral for your care. Within 24 hours, we’ll contact you to confirm receipt and initiate the next steps.

Assessment
Our Patient Care Navigator will reach out to you to schedule your initial assessment and first few visits, and update you on the status of any authorization received or needed from your insurance company. They will also answer any questions you may have about the treatment or scheduling process, serving as your primary point of contact for your scheduling needs.

Start of Care
The first visit, called the Start of Care (SOC), typically lasts 45 minutes to an hour. During this appointment, your provider will comprehensively assess your medical history, details of the wound requiring treatment, past wound treatment experiences, and any ongoing challenges with daily activities or other factors that may impact your care. If you have a caregiver, it is often helpful for them to be present if possible. It allows our provider to understand their role and how they can best support you throughout your healing journey.

Continued Care
Continuing visits - the number of visits will depend on your wound's specific treatment plan and in line with your needs and preferences. As your healing progresses, the frequency of visits and the care pathway will likely change.

Communication
If you have questions or concerns about your wound or your wound care treatment plan in between provider visits, you can simply reach out to your Patient Care Navigator, who will help you connect with your provider or any other member of your care team who can be of assistance.

Support
In rare cases, a wound may be diagnosed as being in a chronic inflammatory state, requiring extended care. When this happens, Woundtech may offer you the option to join our specialized long-term care pathway. This decision is made in consultation with your doctor, other healthcare providers, and your insurance company to ensure comprehensive and continued support.

Discharge
Once your wound has significantly healed, your provider will begin preparing for your discharge. They will work with you to plan a discharge date and ensure you have the necessary support for ongoing care, such as home health visits. Your Primary Care Physician (PCP) will also be notified of your discharge date. This is a milestone and we look forward to celebrating this day with you!​

Wounds We Treat

Woundtech specializes in treating nearly all types of complex wounds. If you’re unsure whether your wound qualifies, your physician can send a referral, and we’ll evaluate your condition for treatment. Common wounds we treat include:
  • Acute skin injury

  • Burns (case by case)

  • Stage 2, 3, 4 or unstageable pressure injuries

  • Trauma wounds

  • Venous or arterial ulcers

  • Diabetic/neuropathy ulcers

  • Wounds requiring debridement

  • Cellulitis with open lesions

  • Wounds requiring NPWT/wound vac
    (evaluate, initiate, and oversee )

  • Pressure injuries: Stage 2 (with education on prevention and progression), Stage 3, Stage 4, or unstageable

  • Surgical and traumatic wounds

  • Non-healing wounds

  • Pyoderma Gangrenosum

  • Full and Partial Thickness Wounds

  • Open Surgical Wounds

  • Hidradenitis Suppurativa

  • Type II Skin Tears or Greater: With 25% partial tissue loss or more
